FYI... you should ask demographic information at the end of a questionnaire. People are most interested and have the most cognitive resources at the beginning, and therefore you should ask the most important questions first. The only exception would be if your most important question are personally sensitive -- in that case you need some soft questions to establish comfort, trust, etc. at the beginning.
